Launch Feedback Assistant
After installing the iOS developer (or public) beta, Feedback Assistant can be launched from the second page of your Home screen. Tap to launch it and sign in with your Apple ID.

Capture diagnostic files
Diagnostic files help Apple investigate the issues you report. When you launch the Feedback Assistant app, diagnostic files are captured automatically and appear as attachments in your feedback report.

You can also capture diagnostic files right after the issue occurred by pressing "Volume Up + Volume Down + Side Button" for 1 to 1.5 seconds. You will feel a short vibration when the diagnostic files are successfully triggered and notification will appear once the diagnostic files have been created.

In order to submit Apple TV feedback, you will need the Feedback Assistant app on an enrolled iOS device or iPadOS device. After installing the iOS or iPadOS developer (or public) beta, Feedback Assistant can be launched from the Home screen.

Sign in with your Apple ID
Sign in with the same Apple ID that you used to register for the Apple Beta Software Program.
Pair your devices
To file feedback on an Apple TV, start a new feedback, select “tvOS” from the list of topics, and then select your Apple TV from the list of devices. Make sure your Apple TV is nearby and running the latest public beta, and your iOS or iPadOS device has Bluetooth turned on.
